PROVISION OF A FLOATING SUPPORT SERVICE

Floating support is a key service in Hounslow’s Independent Living Programme2

. The floating

support provider will be expected to have a whole systems approach, commit to our locality?based way of working within our communities, and develop strong relationships with housing,

health, social care and VCSE partners, contributing to reducing health and social inequalities.

The purpose of the Floating Support service is to improve the quality of life for vulnerable

adults living in the borough through the delivery of relational one to one personalised

preventative support. Support will be outcomes-focused and enabling – doing with, not for.

The service will provide assertive and practical support to residents who are likely to

deteriorate or isolate themselves if no proactive support is given, resulting in crisis, the loss

of independence and poor outcomes, e.g., homelessness, avoidable care packages and care

home placement, non-elective hospital admission, etc.
