Full description
Islington Council intends to award a contract to the existing provider following Provider Selection Regime (PSR) Direct Award C for the provision of a specialist Nurse Outreach service for people experiencing rough sleeping in the London Borough of Islington.
The service will deliver community nursing health assessments and direct clinical interventions, including assertive street outreach and in‑reach into hostels and day centres. The outreach nurse will work as part of the wider rough sleeping multi‑disciplinary team to support individuals with complex physical health needs, facilitate access to primary and secondary healthcare, and help reduce avoidable hospital admissions by addressing barriers to mainstream health services.
The contract is intended to run from 1 April 2026 to 31 March 2027, with an option to extend for a further 12 months to 31 March 2028.
The approximate lifetime value of the contract is £145,000 (excluding VAT) over the maximum contract term.
