Description
Additional capacity required for P1 discharges.
The investment provides short term bridging capacity delivered by Care Assistants to support Pathway 1 discharges from JPUH and GYW community hospitals. This capacity enables medically optimised patients to return home on the day they are ready, by providing up to 72 hours of safe, short term support until NCC/SCC reablement teams commence care.
The service offers:
- Flexible single assist and double assist Care Assistant capacity
1 x Double Assist Team (2 Care Assistants)
1 × Single Assist Teams (1 Care Assistant)
- 7 day a week coverage, 7am until 10pm
- Rapid mobilisation and responsive workforce
- Daily coordination with acute discharge teams and social care
