This service includes the Provision of structured education for patients newly diagnosed with type 2 diabetes. This offers a one-off four hour session offered to patients newly diagnosed with Type 2 diabetes. It offers support and understanding of their condition and effective ways to manage their own health, including exercise and healthy eating. This is offered as a virtual session or a face to face session. It is for small groups, so that the patients interact with the trainer and each other. It is designed to help patients to feel less alone and isolated at a challenging time.
Additional information: The NHS Hertfordshire and West Essex Integrated Care Board was formed on 1st July 2022 and incorporated the previous Clinical Commissioning Groups which covered the Herts and West Essex footprint. Since its incorporation, the ICB has been reviewing its services and contracts to ensure equity of services for patients.
This short-term contract will allow the ICB to undertake a robust review of the service and consider appropriate procurement options in line with relevant legislation.
