- Screening and risk assessment of individuals
- Delivery of programmes in relation to Weight Management, Physical Activity and Falls Prevention, Smoking(specialist and community), Mental Wellbeing (brief intervention) as well as Innovation Projects.
- Case management of participants and tracking of outcomes via an appropriate IT system.
