- Photograph identifying the location.
- General site and survey information
- Survey results including asbestos type results.
- Photographs of the locations of Asbestos material
- Drawing plan indicating the location of Asbestos and the location of labels
- Condition, recommended actions,
- Analysis results
- Asbestos register summary
- Updated data spreadsheet to be completed as per the required format (ACM Import Data attached Appendix) to enable upload onto UHDB
Trust Plant FM system.
- Reports and ACM import data sheet are to be provide to the Trust within 21 days of completed surveys.
