"The services to be provided under this contract are for short term, discharge to assess, care home placements for patients being disharged from hospital via pathway 2.
The contract is intended to be awarded to an existing provider following direct award process C.
The contract is intended to cover services provided from 1st April 2024 to 31st March 2025, and the approximate lifetime value of the contract is £479,136"
