Description
A Healthy Lifestyle Caseload Management System is required to support the work of the Wellbeing Service within Portsmouth City Council. The Portsmouth Wellbeing Service offers support to residents to address multiple unhealthy risk factors, such as smoking, physical inactivity, obesity, unhealthy blood pressure and alcohol consumption. The Wellbeing Service in an inhouse service within Portsmouth City Council which screens all clients for smoking status, BMI (weight and height), physical activity levels, alcohol consumption - as well as using the Edinburgh Warwick scale to support with emotional wellbeing.
The Wellbeing Service engaged with over 2,500 individuals (referrals of 5,000 individuals) in the 2023/24 financial year referred from the hospital (including maternity services), GPs and self-referrals.
