NHS Cheshire and Merseyside ICB - Sefton Place intend to award a 1 year contract via Direct Award C to incumbent provider to deliver Reablement advice & signposting, Every Child Matters forum (Children & Young People), BME Support, High Intensity Service Users Support, Health & Well being Trainers x4, Health & Well being Development & Support Officer & Hospital Discharge services in Sefton.
The contract will start on the 1st of April 2026, expire on 31st March 2027 and has an annual contract value of £528,540.
